Medical Info Program Protects Seniors

Medical history form can save a life in an emergency.

For seniors and individuals with serious medical conditions, Southampton Hospital has launched a new program to provide essential medical information to emergency personnel who respond to home emergencies.

The free "Important Medical Information" program provides a container and a form for medical history, including existing medical conditions, allergies and medications currently being taken. The container, which resembles an oversized pill bottle, is placed in the refrigerator and a magnet is stuck to the outside of the fridge. A decal will be placed on the front door in plain view so responding emergency personnel will know to look for the container.

To receive a free container, email pegan@southamptonhospital.org or call (631) 726-8700 ext. 6.
